The decision to place a loved one in an extended care and rehab facility is never easy; however, we will do our best to make this transition as smooth as possible.
Admission requirements at Lakeview Extended Care and Rehab are the same for every resident. To be admitted to Lakeview Extended Care you must have the recommendation of a licensed physician and the approval of our Admissions Committee. Once the admission has been approved, our staff will work with you to arrange an efficient transfer from the hospital, home or another facility.
Our interdisciplinary team (Physicians, Nurse, Social Worker, Dietician, Therapist, Activity Director, and Certified Nursing Assistants) are committed to developing a plan of care that will meet the physical, emotional and social needs of our new resident. All programs and services are individualized.
Family orientation begins on the day of admission. We hope by sharing information and answering questions we will successfully meet your expectations and make this a comforting experience.
