Welcome to Heartland Health Care Center - Crestview. Heartland is located on 36th Street directly off of Interstate 131 and provides skilled nursing and rehabilitation services to those patients transitioning from hospital to home. Only 5 miles from the new Metro Hospital campus in Wyoming, our patients have access to many medical services without having to drive in to downtown Grand Rapids. Our team of highly trained therapists provide goal oriented physical, occupational and speech therapies in a spacious, newly renovated rehabilitation area. Patients also enjoy choosing delicious meals from our select menu and are served in one of three attractive dining rooms.
